Job summary

Qualifications

  • High school diploma or GED required.
  • Bilingual in Spanish with both oral and written proficiency.
  • Analytical skills to assess patient needs and plan navigation.
  • Ability to communicate with diverse audiences including patients and staff.
  • Strong organizational and data management abilities.
  • Proficient typing skills for documentation.

Responsibilities

  • Coordinate referral appointments with health professionals (genetic counselor, nutritionist, psychiatrist, social workers).
  • Help patients access support services including finances, transportation, and support groups.
  • Triages for needed services across all product lines.
  • Accompany patients to medical visits and coordinate follow-up care.
  • Provide timely support and education to patients and families.
  • Coordinate education and counseling about disease and treatment.
  • Enhance patient satisfaction by addressing support needs during illness.
  • Participate in quality assurance activities.
  • Collect and report information to meet grant requirements.
  • Engage in ongoing education and networking on navigation for patients with language barriers.
  • Participate in outreach programs targeting underserved patients.
  • Speak publicly, in Spanish, at outreach programs serving at-risk populations.
