Oncology Consultants - Business Office is seeking a Community Health Worker to support patients through the cancer care journey. You will educate patients before visits, follow up on health plans, and build trusted relationships with patients and families.

You will also connect patients with community resources and assist with service enrollment. The role emphasizes health literacy, cultural mediation, and care access, with 0–25% travel and bilingual language skills preferred.

Qualifications

  • High school diploma or GED required.
  • Current Community Health Worker Certification required.
  • Bilingual Spanish or Vietnamese preferred.
  • Travel 0–25% of the time.

Responsibilities

  • Serve as a non-clinical member of the oncology care team performing independent, interdependent, and dependent support activities.
  • Prepare and educate patients prior to clinic visits regarding care plans and expectations.
  • Follow up with patients on health management plans and established goals.
  • Develop trusting and supportive relationships with patients and families throughout the cancer care journey.
  • Advocate for patient needs and help coordinate appropriate services and support.
  • Connect patients to health-related services and community resources based on care plans and identified social determinants of health needs.
  • Assist patients with accessing services, including appointment accompaniment and enrollment form support.
  • Facilitate communication among patients, families, care teams, and community-based organizations.
  • Promote appropriate utilization of available health and social support resources.
  • Document patient interactions, activities, and outcomes in medical record and care coordination platforms.
