Job summary

North East Medical Services seeks a Health Education Program Coordinator to develop bilingual health education programs and materials, counsel patients, and coordinate outreach across multiple communities in San Jose, CA. The role requires collaboration with clinical teams and community partners.

The ideal candidate will hold a public health degree, have NBC-HWC training or eligibility, and experience serving underserved populations, with a focus on prevention and education.

Qualifications

• Bachelor’s degree in public health or a related field (preferred).
• NBC-HWC Health & Wellness Coach training or ability to obtain within 6 months is required.
• CHES certification preferred.
• Two years or more of experience in community health education or related field preferred.
• Experience with underserved, immigrant, low-income and/or Asian communities preferred.
• Current Basic Life Support (BLS) certification is required.

Responsibilities

• Develops bilingual health education programs and activities for preventative health care and related topics.
• Develops curricula, toolkits, and health education materials.
• Provides group classes and/or one-on-one health education services.
• Acts as liaison with community agencies and schools for outreach programs.
• Manages Health Education centers.
• Counsels patients on diabetes, hypertension, asthma, smoking cessation, and more.
