Job summary

People’s Community Clinic in Austin, TX is seeking an Advanced Practice Nurse Practitioner to join a care team providing comprehensive primary care to an underserved population. You will assess, diagnose, treat, and coordinate care across services with emphasis on self-management and culturally competent care.

This full-time role requires collaboration with RNs and physicians, documenting in Epic, supporting quality metrics, and ongoing education. Competitive benefits apply.

Qualifications

• Graduation from an accredited 4-year college or university.
• Graduation from an accredited master’s program with major coursework in public health or a related field is preferred.
• 1–3 years’ experience in project or grant management.
• 1–3 years’ experience in non-profit organizations, healthcare, or a related field.
• Bilingual (English/Spanish) preferred.

Responsibilities

• Provide direct medical care for patients according to established standards of care.
• Provide culturally competent care to a diverse and underserved patient population.
• Provide comprehensive primary care services, including exams, screenings, immunizations, and management of conditions.
• Conduct Medicare Annual Wellness Visits in a clinic per CMS guidelines.
• Identify preventive needs and develop personalized care plans with patients.
• Document encounters accurately in the EHR and submit billing codes promptly.
