Cencora is seeking a Nurse Case Manager for Canada market supporting patient and provider services and therapy funding coordination. Role involves coordinating with healthcare providers, documenting patient interactions, and ensuring adherence to program protocols.

Ideal candidates will be licensed nurses with experience in healthcare and case management, fluent in French and/or English, and capable of leading teams and improving processes within a dynamic environment.

Formación

• RN or RPN with active licensure and healthcare/case management experience.
• Experience in healthcare, pharmaceutical, or insurance sectors.
• Bilingual (French/English) may be required depending on location.
• Knowledge of change management and QA processes.

Responsabilidades

• Provide information on Patient Support services to patients and health care providers.
• Assist patients and caregivers with assessments related to Patient Support services.
• Coordinate with health care providers and funding sources to secure therapies.
• Document patient information in the electronic patient record and follow case-management protocols.
• Collaborate with reimbursement specialists and physicians on funding options for therapies.
