Job summary

Wheeler is seeking a Bilingual Crisis Clinician to provide crisis assessment, intervention and stabilization for youth from birth to 17 years in Hartford, CT. The role involves field response, community-based planning, and coordination with departments to prevent hospital admissions.

Qualifications

• Master’s degree in Marriage and Family Therapy, Social Work, or Counseling, or a Doctoral degree in Psychology.
• Must hold LMSW, LCSW, LMFTA, LMFT, LPCA, LPC or licensed or license-eligible Psychologist in the State of Connecticut.
• Valid Driver's License is required for community-based work.

Responsibilities

• Conducts program specific screening and evaluation per DSM; develops and documents clear clinical formulations and treatment plans.
• Triages clinical emergencies and follows safety protocols; uses evidenced based practice models.
• Provides mobile service in community settings (homes, schools, hospitals, etc.) and may transport clients.
• Facilitates referrals and coordinates care across services and agencies to address client needs.
• Communicates effectively with clients, families, and providers; attends required meetings.
• May coordinate with EMPS teams to provide intensive follow-up care and stabilization for high-need youth.
