Qualifications

Responsibilities

• Provide crisis de-escalation/intervention and assessment, safety planning, and stabilization.
• Coordinate discharge/transition services, including referral and linkages.
• Complete an individualized crisis plan for each client.
• Establish and maintain client records and electronic documentation.
• Record service data as required for statistical reporting.
