Job summary

Oak Street Health is seeking a Medical Social Worker to contribute to care teams by assessing psychosocial needs and coordinating comprehensive care for high-risk patients. You will collaborate with interdisciplinary teams, advocate for patients, and connect them to necessary community resources.

Ideal candidates hold an MSW from a CSWE-accredited program, are minimally licensed in the relevant state, and possess multilingual abilities with proficiency in EMR systems.

Qualifications

• MSW degree from a CSWE-accredited program.
• Licensed or minimally licensed in the state where OSH Center is located.
• Fluency in Spanish, Polish, Russian, or other languages used in the community.

Responsibilities

• Provide psychosocial assessments and care coordination for high-risk patients.
• Collaborate with interdisciplinary teams to develop care plans.
• Advocate for patients and connect with community resources.
• Educate patients and families on care options and goals.
• Document MSW interventions in a timely manner.
• Respond to crises and provide culturally competent care.
