Oak Street Health is seeking a Medical Social Worker to support psychosocial needs, provide assessments, and coordinate care for high-risk patients and their caregivers. You will work with an interdisciplinary team to identify goals, arrange resources, and advocate for patients across the care continuum.

The role requires an MSW from a CSWE-accredited program, licensure in the appropriate state, and language fluency where needed. Strong EMR and Microsoft/GSuite skills are essential.

Qualifications

• MSW degree from CSWE-accredited program.
• Minimally licensed in the state where OSH Center is located.
• Fluency in languages spoken by the communities served (where necessary).
• Proficient computer skills including Microsoft, GSuite, and EMR.

Responsabilités

• Provide comprehensive assessments, advocacy, and care coordination for high-risk patients and caregivers.
• Collaborate with interdisciplinary team to identify goals and execute care plans.
• Refer to community resources to meet health and psychosocial needs.
• Navigate insurance benefits with the PRM to maximize plan benefits.
• Document interventions clearly for visibility across the care team.
• Educate patients on care options and goals of care at all stages.
