Qualifikationen

• Master of Social Work degree from a CSWE-accredited program.
• Minimally licensed in the state where OSH Center is located.
• Fluency in Spanish, Polish, or Russian or other relevant languages.

• Provide assessments, advocacy, and care coordination for high-risk patients.
• Coordinate with care teams to improve health outcomes and reduce hospitalizations.
• Educate patients on resources, benefits, and end-of-life planning as needed.
• Document interventions clearly in the EMR for team visibility.
• Respond to crises with prioritized, multitaskable action.
