Job Description

This is a full-time, work-from-home position. The candidate must be located in the Chicago, IL area due to regular local travel (80%) for in-person patient appointments.

Fluency In Spanish Required.

As a Field Case Manager, you will work closely with treating physicians/providers, employers, customers, legal representatives, and the injured/disabled person to create and implement a treatment plan that returns the injured/disabled person back to work appropriately, ensures appropriate and cost-effective healthcare services, and promotes maximum medical recovery. Responsibilities include:

1. Demonstrating knowledge, skills, and competency in case management standards of practice.
2. Advising on treatment plans with advanced knowledge of injuries, medications, and job duties.
3. Interviewing disabled persons to assess recovery and injury origin.
4. Collaborating with physicians to create tailored treatment plans.
5. Working with employers and physicians to modify job duties for early return to work.
6. Evaluating and adjusting case goals based on progress.
7. Managing workload independently and effectively.
8. Attending appointments, preparing status updates, and other duties as assigned.

Qualifications

• Associates or Bachelor’s Degree in Nursing or related field.
• 2+ years of clinical practice preferred; workers’ compensation experience is a plus.
• Ability to advocate effectively, work independently, and possess basic computer skills.
• Active RN license in good standing.
• Reliable transportation and internet; ability to travel locally.
