Job Description

This is a full-time, work-from-home position. Candidates must be located in the Frankfort, IL area due to regular local travel (80%) for in-person patient appointments.

Role Overview

As a Field Case Manager, you will collaborate with physicians, employers, customers, legal representatives, and injured/disabled persons to develop and implement treatment plans that facilitate recovery, ensure appropriate healthcare, and promote return to work and optimal functioning. Responsibilities include:

• Applying case management standards of practice
• Advising on treatment plans based on injury types, medications, and job duties
• Assessing recovery through interviews
• Collaborating with healthcare providers to tailor treatment plans
• Modifying job duties to facilitate early return to work
• Managing workload independently
• Attending appointments, preparing status updates, and other duties as assigned

Qualifications

• Associates or Bachelor’s Degree in Nursing or related field
• 2+ years clinical practice preferred; workers’ compensation experience a plus
• Active RN license in good standing
• Strong advocacy and communication skills
• Proficiency in basic computer skills (Excel, Word, Outlook)
• Reliable transportation and internet
