Job summary

LCH Health and Community Services is looking for a Registered Nurse (RN) in Kennett Square, PA, dedicated to providing high-quality patient care through chronic disease management and care coordination. Qualifications

Responsibilities

  • Provide triage support (phone and in-person).
  • Manage chronic care patients and support population health initiatives.
  • Educate patients and families on chronic conditions and treatment plans.
  • Coordinate follow-ups, referrals, and transitions of care.
  • Support women's health services, including prenatal/postpartum care.
  • Administer vaccines and perform point-of-care testing.
  • Collaborate with providers and interdisciplinary care teams.
  • Participate in case management and community outreach.
  • Maintain accurate documentation and ensure HIPAA compliance.

About the Role

We’re seeking a Registered Nurse (RN) to support patient care through chronic disease management, triage, and care coordination. This role focuses on high-risk patient populations and works closely with providers and care teams to deliver patient-centered, team-based care across primary care, women’s health, and pediatrics.

This position is eligible for the Nurse Corps Loan Repayment Program.

What You’ll Do

Schedule Expectations

Full-time role with travel between LCH locations as needed.

What You Bring
  • Bilingual English/Spanish required
  • Associate or bachelor’s degree in nursing
  • Current or eligible RN license in Pennsylvania
  • CPR certification required
  • Community or public health experience (preferred)
  • Experience working with diverse populations (preferred)
  • Experience with EHR systems and Microsoft Office
