La Clinica de Familia Inc is looking for a collaborative Integrated Care Behavioral Health Provider in Las Cruces, New Mexico. This role assists primary care providers in managing the health of enrolled patients by offering consultation services and brief interventions in a team-based environment.

The candidate must possess a Master's Degree in Social Work or a related field, hold a relevant New Mexico license, and be bilingual in English and Spanish. Comprehensive benefits including health, dental, and 401(K) matching are provided.

Qualifications

• Must have a relevant New Mexico license (LMSW, LMHC, LCSW, LPCC, LMFT).
• Experience in behavioral health and integrated care.
• Familiar with clinic-based service delivery.

Responsibilities

• Provide consultation services to patients referred by primary care.
• Assist PCP in managing overall health of the enrolled population.
• Deliver brief consultation-based services in the primary care clinic.
