Qualifications

• Master's degree in Social Work or related field required.
• Must possess NM licenses LMSW, LMHC, LCSW, LPCC, or LMFT.
• Bilingual English/Spanish is required for the role.

Responsibilities

• Provide consultation services to patients referred by the primary care team.
• Deliver brief, consultation-based services in the primary care clinic using an integrated care model.
• Collaborate with the PCP and healthcare team to manage the enrolled population's psychosocial/behavioral health needs.
