Job summary

Child and Family Guidance Center (CFGC) in the San Fernando Valley seeks a full-time Bilingual Case Manager for the Northridge-based Full-Service Partnership program. You will provide in-home case management to families receiving mental health treatment, reporting to the Community Resource Services Director.

Responsibilities include completing psychosocial assessments, developing treatment plans with families and clinicians, coordinating with schools and community agencies, and educating parents

Qualifications

• BA/BS in Social Work or related field.
• Two+ years of case management or related experience.
• Experience locating community resources for low-income families.

Responsibilities

• Accepts case assignment and reviews records to clarify the case management request.
• Conducts in-home psychosocial assessments with families.
• Develops and monitors a CRS case management plan addressing family and treatment needs.
• Interfaces with community agencies to locate services that fit the family’s needs.
• Educates parents on educational services and monitors delivery of supports.
• Communicates progress with the treatment team and completes timely documentation.
• Drives own vehicle as needed.
