Job summary

Child and Family Guidance Center (CFGC) in Los Angeles is seeking a full-time Coordinated Care Case Manager to provide comprehensive case management across CalWORKs, HFW, FSP, and other programs. The role requires flexibility to work with diverse client populations and across agency priorities, with an on-site schedule.

Responsibilities include developing individualized plans, coordinating care with therapists and other team members, helping clients access education, job training, and community

Qualifications

• Bilingual Spanish is required.
• Ability to function in a multi-disciplinary team.
• Ability to maintain confidentiality and exercise good judgment.

Responsibilities

• Maintain an active caseload across CalWORKs, HFW, FSP, TAY, and other programs.
• Develop, implement, and monitor individualized case management plans.
• Coordinate with therapists, psychiatrists and treatment teams for continuity of care.
• Help clients build coping, organizational, and life skills.
• Assist clients in navigating social services and community appointments.
• Connect clients with educational, vocational, and training resources.
• Participate in planning meetings and multidisciplinary team meetings.
• Maintain documentation per DMH guidelines and meet productivity standards.
• Attend required trainings and team meetings.
• Work full-time on-site (40 hours/week). Starting salary $24–$25/hour.
