• Bachelor's degree in Behavioral Science, Social Science, or a closely related field.
• Bachelor's degree in any field plus 1 year of human services related experience.
• Associate's degree in Behavioral/Social Science plus 2 years of human services experience.
• Three years of human services related experience.

Responsibilities

• Match community needs with federal, state, and local programs or resources through case management.
• Screen applicants for long-term community-based care and nursing homes; assess eligibility and adjust benefits.
• Advocate for clients and Aging and People with Disabilities within the community.
