Job summary

Oregon Department of Human Services is seeking a Case Manager (Human Services Case Manager) to assess and coordinate services for older adults and people with disabilities, enabling continued independence and safety. The position is located in Hillsboro, Oregon.

Fluency in English and Spanish is preferred; the agency highly values equity and inclusive service delivery. A valid driver’s license and acceptable driving record are required, with background checks and pre-employment screening

Qualifications

• Valid driver’s license required.
• 3 years of human services related experience or a relevant degree with experience.
• Preference for bilingual (English/Spanish) candidates.

Responsibilities

• Conduct interviews to determine eligibility for long-term care and social services.
• Document and update case information using state systems and tools.
• Develop person-centered service plans and determine appropriate services/referrals.
• Process applications and communicate eligibility decisions within policy timelines.
• Monitor service plans with participants and coordinate with agencies and providers.
• Collaborate with participants, medical providers, and community partners to provide benefits and information.
• Create risk-mitigation plans and provide risk mitigation as needed.
