Job summary

The Oregon Department of Human Services is hiring a bilingual English/Spanish Case Manager to assess applicants and coordinate long-term care services for older adults and people with disabilities in Oregon. You will develop and monitor person-centered service plans, work with medical providers and community partners, and ensure timely eligibility decisions.

This role is based in Hillsboro, OR, with responsibilities across aging, disability, and community support programs.

Qualifications

• Minimum qualifications include a valid driver’s license and acceptable driving record.
• 3 years of human services related experience or an equivalent education pathway.
• Ability to assess needs, coordinate services, and develop person-centered plans.
• Preference for bilingual candidates (English/Spanish).

Responsibilities

• Conduct interviews to determine initial and ongoing eligibility for long-term care, healthcare, social services, and protective services.
• Utilize state systems and resources to document and update case information and long-term care services.
• Assess needs, safety, and risk to develop detailed, person-centered service plans and determine services, referrals, and resources.
• Process applications and communicate eligibility decisions within policy timeframes.
• Contact participants monthly to monitor service plan and safety needs.
• Collaborate with participants, other agencies, medical providers, and community partners to provide benefits and information.
• Create person-centered risk mitigation plans and provide risk mitigation as needed.
