Job summary

The Mental Health Association (DSH) is seeking a Case Manager to provide trauma-informed, person-centered support for individuals in substance use and/or mental health recovery with homelessness history.

The role focuses on helping participants transition into studio apartments within congregate housing and continues with ongoing support to achieve goals such as stable income, medical access, and employment.

Qualifications

• Bilingual candidates strongly encouraged (English/Spanish).
• Valid driver's license with adequate insurance and reliable transportation.
• Ability to work independently and as part of a team.
• Strong organizational, communication, and interpersonal skills.
• Capacity to adapt to evolving participant needs.
• Must be at least 21 and able to obtain CPR and First Aid certifications.
• High school diploma or equivalent required.
• Minimum of six months of relevant experience.

Responsibilities

• Coordinate documentation for program entry (homelessness verification, assessments, eligibility, IDs, social security, income verifications).
• Complete Adult Comprehensive Assessments, updates, and service plans.
• Develop Individualized Action Plans (IAPs) with participant input and strengths.
• Implement IAPs and document progress.
• Develop and revise safety plans with participants and team.
• Assist participants with transitioning to stable housing.
• Maintain communication with Chestnut Crossing staff.
• Support tenants in housing responsibilities and regulations.
• Assist participants in seeking employment, training, or education.
• Educate participants on accessing public benefits and resources.
• Encourage community integration and participation in programs.
