Qualifications

• Must have 2+ years of experience as a RN in Home Health.
• Reliable transportation and Texas Drivers License required.
• Current TB Skin Test needed.

Responsibilities

• Perform case management and supervised visits.
• Conduct comprehensive assessments during initial admission.
• Document patient observations at each visit.
