Job summary

Frontpoint Health is seeking a Bilingual Home Health RN Case Manager (PRN) to provide healthcare services in patients' homes. As part of your role, you will perform assessments, administer medications, and help maintain patients' independence through skilled care.

The ideal candidate will have a Texas RN license, at least 2 years of experience in home health, and current CPR certification. Reliable transportation and a TB skin test are also essential for this position.

Qualifications

• 2+ years' experience as a RN in Home Health.
• Reliable transportation.
• Current TB Skin Test.
• Texas Driver's License and Auto Liability Insurance.

Responsibilities

• Perform skilled and comprehensive assessment of the patient during the initial admission.
• Provide proper patient education for home health care procedures and strategies.
• Maintain confidentiality of patient PHI through HIPPA law.
