Qualifications

• Current RN license in Texas is required.
• Minimum 1 year of home health experience preferred.
• Strong clinical skills and disease management knowledge.
• Excellent communication and interpersonal abilities.
• Ability to work independently and as part of a team.

Responsibilities

• Perform skilled nursing services under supervision.
• Observe, record, and report patient conditions to supervisor.
• Prepare clinical notes and add to the chart weekly.
• Communicate changes in patient status to physician and team.
• Manage patient care orders and adhere to POC standards.
• Attend in-service trainings and maintain team education.
• Coordinate holds, approvals, and scheduling for visits.
• Triage calls and coordinate with MDs and care team.
• Follow admits, plan therapy, and document progress.
• Review 485/Lock OASIS timely and assist with referrals.
