Responsibilities

• Assists with activities of daily living, including bathing, dressing, grooming, mobility assistance, and meal preparation.
• Provides light housekeeping as needed and transports clients to appointments and outings.
• Delivers personalized care per the individualized care plan.
• Reports changes in client condition promptly to the branch supervisor.
• Documents visits and tasks in the home care application.
