Responsibilities

• Match caregivers with clients based on individual needs and preferences.
• Schedule and assign home care aides to ensure timely and efficient service delivery.
• Serve as the primary point of contact for clients, families, and caregivers, addressing inquiries, resolving issues, and providing updates.
• Monitor the quality of care and ensure client satisfaction by conducting periodic follow-ups.
• Utilize HHA Exchange software to manage client schedules, track caregiver assignments, and ensure compliance with service plans.
• Input and maintain accurate client and caregiver records, ensuring data integrity.
• Maintain comprehensive and up-to-date documentation of client interactions, schedules, and service plans.
• Work closely with the care management team to ensure seamless communication and coordination of services.
• Assist with recruitment and onboarding of caregivers as needed.
