Job Summary:

The Home Care Intake Specialist is the first point of contact for patients and plays a key role in ensuring a smooth and welcoming experience. This position is responsible for gathering patient information, verifying insurance and patient demographics, and preparing the necessary documentation for clinical staff. The ideal candidate is detail-oriented, organized, and has strong interpersonal and communication skills.

Key Responsibilities:

  • Contact patients via telephone in a professional and courteous manner
  • Collect and verify patient demographics and insurance information
  • Obtain necessary authorizations, referrals, and consents
  • Accurately enter patient information into the electronic medical record (EMR) system
  • Documenting patient interactions and communication
  • Manage appointment scheduling/provider routes
  • Answer incoming calls, respond to inquiries, and redirect calls appropriately
  • Maintain confidentiality of patient records in compliance with HIPAA
  • Collaborate with clinical staff to ensure smooth patient flow and accurate handoffs
  • Perform administrative duties as needed (scanning, faxing, filing, etc.)
  • Perform other duties as assigned

Qualifications:

  • High school diploma or equivalent required
  • Medical office or healthcare customer service experience preferred
  • Familiarity with EMR systems (DocNow) a plus
  • Strong attention to detail and organizational skills
  • Excellent verbal and written communication skills
  • Ability to multitask in a fast-paced environment
  • Knowledge of medical terminology and insurance processes is a plus
  • Bilingual (Spanish)

Physical Requirements:

  • Ability to sit or stand for extended periods
  • Frequent use of computer and office equipment
