Job summary

JPS Health Network seeks a Clinical Pharmacist for ambulatory HIV/Hepatitis C care to work with a multidisciplinary team. You will lead pharmacotherapy, conduct drug utilization reviews, and monitor therapy and interactions in outpatient settings.

The role includes transitioning patients from hospital to HIV clinic, developing optimized regimens, and serving as a preceptor for students and residents while supporting care quality and safety.

Qualifications

• Pharm.D. or equivalent from an accredited College of Pharmacy.
• 2 year ASHP residency in HIV or 5 years direct HIV patient care.
• Pharmacy license from Texas Board of Pharmacy or eligible.
• Preceptor with Texas Board of Pharmacy or complete within 3 months.
• Specialty Board Certification or complete within 24 months.
• Immunization Certification or complete within 6 months.
• BLS certification or complete within 3 months.

Responsibilities

• Provide clinical leadership for pharmaceutical care in HIV/Hepatitis areas.
• Serve as a resource for outpatient and inpatient HIV pharmacotherapy.
• Transition patients from hospital to HIV Clinic with appropriate therapy.
• Develop and optimize regimens via collaborative practice agreements.
• Perform drug utilization reviews and monitor for interactions.
• Minimize polypharmacy and create patient medication action plans.
• Precept pharmacy students or residents and participate in education.
• Collaborate with multidisciplinary team in the Medical Home.
• Present interventions and medication plans to care team.
• Provide dosing, monitoring, and drug information to clinicians.
