Qualifications

  • Effective verbal and written communication skills in English and Spanish.
  • Must be licensed (unrestricted) and registered as a Pharmacy Technician or willing to apply for temporary registration as a Pharmacy Technician Trainee in the state of employment.
  • High School Diploma or equivalent required; Bachelor’s degree in social work preferred.
  • Two years of experience in a health care or pharmacy setting preferred.
  • Experience working with clients living with HIV/AIDS or at-risk patients.
  • Strong interpersonal skills to affect positive outcomes.

Responsibilities

  • Manage a caseload of 350–400 patients from diverse backgrounds.
  • Contact patients monthly to ensure medication adherence.
  • Serve as a liaison between patients and healthcare team.

Essential Functions & Responsibilities
  • Manage a case load of 350-400 patients of a diverse population living with mental health issues, substance abuse issues, and/or diseases such as HIV, and psychosocial stressors.
  • Contact patients monthly to ensure adherence to medication regimen and encourage management of healthcare through proactive work/support.
  • Develop relationships with patients built on integrity and loyalty. Meetings patients where they are through care skills such as empathy, active listening, courtesy, and understanding.
  • Serve as a liaison between patients and healthcare team, triaging medical, psychological, and social issues with pharmacists, doctors, clinics, and/or appropriate provider(s).
  • Collaborate with providers to help clients overcome barriers to care.
  • Process medications for fulfillment, sort queued items and match refills for processing.
  • Locate refills that have not been obtained from initial requests and shipped packages not delivered at expected time.
  • Conduct follow-ups with unreachable patients and patients with an overstock of medication.
  • Record all communication/ events concerning any/all communication with patients, staff, clinics, doctors and insurance agencies at all times.
  • Implement and adhere to the policies and procedures for Specialty HIV program.
  • All other duties assigned by Program Manager of Client & Patient Services.
Physical Requirements / Working Conditions / Behavioral Expectations
  • Ability to utilize equipment including but not limited to: Telephone, Computer, Designated software(s), Basic office Software (MS word, Excel), Printers, Calculators, Facsimile Machines and other Pharmacy Equipment.
  • Ability to walk, stand, sit and reach on a frequent basis.
  • 15% Travel (within state and out of state).
  • Ability to work independently and in a team environment.
  • Ability to listen, record, and type data quickly and accurately.
  • Ability to collect and analyze data and verbal information to reach logical conclusions.
