Qualifications

• Associate's degree related to health or human services.
• At least 1 year of related experience in community services or health care front office duties.
• Valid Driver’s License required.
• WIC Nutrition Assistant certifications within 1 year if assigned to WIC.

Responsabilités

• Provide assistance and support services to clients and health staff.
• Perform case management and coordinate services for clients.
• Determine initial and ongoing eligibility for programs.
• Attend staff meetings and trainings.
• May act as interpreter for non-English speaking clients.
• Maintain accurate documentation and reports.
