Job summary

Sea Mar Community Health Centers is seeking an Integration Specialist to provide Health Home services across home, community, clinic and inpatient settings. The role includes care coordination, health promotion, and referrals to community resources, with regular follow-up and documentation.

Qualifications include a BSW/BA in a related field, experience with chronic conditions, and strong communication skills.

Qualifications

• Ability to work effectively with diverse populations and interdisciplinary teams.
• Strong organizational and communication skills; professional boundaries.
• CPR certification within 90 days of hire and ongoing maintenance.

Responsibilities

• Review screenings and coordinate care with service providers to support patient goals.
• Create a health action plan (HAP) with long- and short-term goals and actionable steps.
• Provide six core Health Home services: health promotion, family support, care coordination, transition care, referrals, and care management.
• Maintain ongoing communication with care team and document contacts per policies.
