Job summary

Sea Mar Community Health Centers seeks an Integration Specialist (BA) to support Health Home services. You will work with patients in homes, communities, and clinics, coordinating care across medical, behavioral health, and social services to help patients manage chronic conditions.

The role emphasizes motivational interviewing, care planning, and regular follow-ups with patients and families, supporting an interdisciplinary care team in a fast-paced environment.

Qualifications

• BSW or BA/BS in Human Services or related field.
• Experience in social service case management or care coordination.
• Ability to work with interdisciplinary care teams including medical and behavioral health providers.

Responsibilities

• Review screenings and coordinate initiation of Health Action Plan with patient and care team.
• Provide six core Health Home services: health promotion, care coordination, and referrals.
• Monitor patient progress and adjust care plans with providers as needed.
• Maintain documentation of calls, visits, and encounters per policy.
• Collaborate with community resources and case managers for complex cases.
