Job summary

Sea Mar Community Health Centers in Washington seeks a Behavioral Health Specialist to provide primary community support and case management for mentally ill, homeless, and substance-using clients. You will work as part of a multidisciplinary team to coordinate services, conduct intake screenings, and develop individual service plans.

Requirements include an Associate’s degree in Behavioral Health or related field, two years of relevant experience, licensure eligibility as an Agency Affiliated

Qualifications

• Associate degree in Behavioral Health, Health Sciences, or related field.
• Active required Agency Affiliated Counselor Registration or eligibility within 90 days of hire.
• Must have and maintain TB test; valid State Driver’s license; annual influenza vaccine.

Responsibilities

• Conduct intake screenings and determine need for case management.
• Develop and implement Client Service Plans for each client.
• Make referrals to appropriate agencies and provide information for self-referral.
• Maintain regular contact with clients including home visits and advocacy.
• Maintain case files and participate in team case reviews.
