Qualifications

• High school diploma or equivalent required; 1 year field operations experience preferred.
• Associate or bachelor’s degree in social work or related field strongly preferred.
• Ability to travel frequently, including evenings and overnights.
• Strong verbal and written communication; objective documentation.

Responsibilities

• Conduct in-person field visits within an assigned geographic region.
• Verify participant residence and document findings.
• Observe wellness indicators and report safety concerns.
• Document visit data and coordinate with supervisors and teams.
• Travel domestically 75%–90%; maintain logs and case notes.
