Job summary

MedZed is seeking a Bilingual Community Health Navigator II in Fremont/Newark, CA. The role is field-based with significant travel and a hybrid/remote work setup. You will manage complex member cases, perform ECM assessments, and support care planning to re-connect members to primary care.

With 2+ years in ECM or related fields, you will conduct outreach via phone and in-person visits, document thoroughly, and ensure compliance with CalAIM and DHCS requirements while meeting engagement metrics.

Qualifications

• High School Diploma or equivalent required.
• 2+ years’ experience in ECM, case management, or community health.
• Demonstrated excellence in documentation and compliance.
• Ability to manage complex psychosocial and medical needs.
• Curious and relentless nature; strong outreach to enroll members.

Responsibilities

• Manage an ECM caseload up to 50+ members.
• Complete initial and follow-up ECM assessments.
• Support development and updates of individualized Care Plans.
• Calls, field visits, and door knock to enroll new members.
• Build and maintain a member panel through outreach.
• Coordinate referrals to health plans, providers, and community resources.
• Document timely, accurate, and compliant case notes in Salesforce Health Cloud.
