Job summary

Pair Team is seeking a Bilingual Field Care Coordinator in Oakland, California to deliver field-based care management services across Alameda County. You will lead in-person outreach, onboarding new patients, and connecting them with clinical, behavioral, and social resources, while addressing barriers to care in the community.

The role requires 1+ year of field-based case management, reliable transportation, and strong cultural fluency.

Qualifications

• 1+ years of field-based (in person) case management.
• Location in or near Alameda County.
• Valid driver’s license and auto liability insurance.
• Reliable transportation for regular field visits.
• Strong understanding of cultural fluency and empathy.
• Experience with Google Suite, Slack, CRM databases.
• HIPAA‑compliant workspace and stable internet connection.
• Proactive, organized with attention to detail.
• Gas reimbursements for on‑site engagement days.

Responsibilities

• Manage a high-volume field caseload with ~30 in-person visits weekly.
• Travel across Alameda County to meet patients where they are.
• Coordinate services and referrals across primary care, behavioral health, and social resources.
• Develop and maintain individualized care plans and document interactions.
• Advocate for member needs within healthcare and social systems.
• Participate in care team meetings to review patient panels.
• Support community events and distribute care packages.
