Job summary

Qualifications

• Bachelor's degree in health and human services field.
• 2+ years of related experience.
• Exceptional verbal/written skills.
• Live in Collier or Charlotte county in Florida.
• Valid state driver’s license and reliable vehicle.

Responsibilities

• Visit Medicaid members in their homes, Assisted Living Facilities, and Long Term Care Facilities; 75-90% local travel.
• Assess and evaluate member needs to establish a member-specific care plan.
• Coordinate services and monitor the care plan against goals.
• Guide members/families toward resources appropriate for their care.
• Ensure services are in the least restrictive setting to achieve wellbeing.
• Plan and implement interventions to meet member needs.
• Interact with payer sources, providers, interdisciplinary teams, and others as required.
