Qualifications

• 2+ years in care coordination, case management, or related health/human services role.
• Proficient in Word, Excel, Outlook; strong communication skills.

Responsibilities

• Conduct telephonic and face-to-face assessments to evaluate members' health, functional, environmental, psychosocial, and support needs.
• Develop and maintain individualized, member-centered care plans reflecting needs and goals.
• Plan and implement interventions, coordinate services, and monitor progress.
