Job summary

You will assess family needs, deliver curriculum-based education to prenatal families and children under 5, provide case management, and coordinate with partners for referrals.

Qualifications

• Must be bilingual in Spanish.
• High school diploma or GED with experience assisting children or families in a community setting.

Responsibilities

• Initiating and maintaining regular contact with families, primarily by traveling to their homes.
• Using curriculum and materials to provide service to prenatal families and children under 5.
• Conducting program outreach to increase referrals.
• Administering standardized assessments during home visits.
• Establishing trusting relationships with families.
• Providing prenatal health education and parenting support.
• Assisting families to optimize home environments for child well-being and safety.
• Timely reporting and case documentation in an electronic database.
