Job summary

Trinity Health Mid-Atlantic is seeking a Family Support Specialist for the Healthy Families of Rensselaer County program. This full-time role involves conducting intensive home visiting to promote child well-being and prevent abuse or neglect, with Spanish fluency required and travel across communities as needed.

The position includes mandatory supervision, ongoing trauma-informed training, and reporting in HFNY systems.

Qualifications

• HSD/GED required.
• Fluent in Spanish required.
• Experience working with children and families.
• Valid NYS driver's license required.
• Valid automobile insurance required.
• Willing to travel up to 400 miles per month with mileage reimbursement.

Responsibilities

• Establishes trusting relationships with at-risk families via regularly scheduled home visits from intake through enrollment up to 5 years per family as per HFNY standards.
• Participates in weekly mandatory supervision focusing on administration, clinical and reflective strategies.
• Ensure accurate and timely reporting and case documentation in HFNY databases and MIS within required timeframes.
• Maintain at least 75% home visit rate and up to 25 families on caseload as per HFNY standards.
• Provide continuous outreach to families and community referral sources to maintain caseloads and program capacity.
• Comply with NYS mandated reporter training and HFNY policies.
