Job summary

One Brooklyn Health System Inc. is seeking a Family Resource & Support Specialist to deliver home-based, culturally responsive services to at-risk families.

You will assess needs, develop service plans with families, and connect them to health and community resources. Responsibilities include home visits, prenatal and parenting education, child development screenings, coordination of events, and ensuring documentation complies with hospital and HFNY requirements; evening and weekend availability

Qualifications

• Experience with high-risk families in a community setting.
• Willing to work evenings, weekends, and some overnight travel.
• Bilingual abilities (Haitian Creole/French/Spanish) for some positions.

Responsibilities

• Initiates and maintains regular contact with families, primarily in their homes.
• Establishes trusting relationships with families and conducts HFNY assessments.
• Supports an average caseload of 20 families and provides health education.
• Provides prenatal and parenting education, and facilitates home-based activities.
• Coordinates events, participates in supervision, and documents activities.
