Job summary

New North Citizens’ Council is seeking a Family Support Navigator in Springfield, MA to provide information, referral, and service navigation to families of individuals with intellectual and developmental disabilities, in alignment with DDS Family Support Guidelines.

The role reports to the Family Services Supervisor and involves coordinating services, developing Family Support Plans, and connecting families with community resources, benefits, and supports, with bilingual English/Spanish

Qualifications

• Associate degree or 3+ years in human services as requested.
• Knowledge of intellectual and developmental disabilities and family needs.
• Ability to assess family needs and develop supportive plans.

Responsibilities

• Provide information, referral, and service navigation to families.
• Develop and maintain Family Support Plans reflecting priorities and values.
• Coordinate services and community resources to promote family independence.
• Maintain ongoing communication with assigned families.
• Document referrals and services per guidelines.
• Collaborate with DDS coordinators and community partners.
