Job summary

New North Citizens’ Council in Springfield, MA is seeking a Family Support Navigator to guide families with intellectual and developmental disabilities through information, referrals, and service navigation.

You will collaborate with the Family Services Supervisor, develop individualized Family Support Plans, coordinate community resources, and help families access benefits while maintaining confidential records and ensuring program compliance.

Qualifications

• Associate's degree or 3+ years in human services with a focus on families affected by intellectual and developmental disabilities.
• Knowledge of intellectual and developmental disabilities and related services.
• Strong interpersonal, communication, organizational, and problem-solving skills.
• Ability to work independently and manage competing priorities.

Responsibilities

• Provide information, referral, and navigation services to individuals and families.
• Develop and maintain individualized Family Support Plans reflecting family priorities and values.
• Coordinate DDS services and flexible funding with DDS personnel and community providers.
• Maintain accurate, confidential documentation of family contacts, referrals, and services.
