Job summary

SGA Youth & Family Services is seeking a full-time Home Visitor to support families in the Chicago area. The role focuses on maintaining a caseload of 20-24 children and pregnant mothers through regular home visits, intake support, and coordinated family activities.

You will work with diverse families, complete essential forms, and coordinate with partners to address family needs. Bilingual Spanish speakers are preferred.

Qualifications

• Minimum 3–5 years of experience working with young children ages 0–3 or with parents.
• Must complete Parents as Teachers Model Certification training.
• Ability to write routine reports and communicate with diverse families.

Responsibilities

• Recruit and maintain a caseload of up to 20-24 children/pregnant mothers.
• Conduct biweekly visits and submit weekly schedules.
• Work with parents to obtain necessary intake documentation.
• Assist in maintaining enrollment rate through outreach and recruitment.
• Develop home visit and activity plans based on child assessments.
• Collaborate with team to plan parent/child activities.
• Maintain confidentiality and accurate data in the program records.
• Complete ASQ screenings to identify special needs and referrals.
• Advocate for families and coordinate with community partners.
