Job summary

Wheeler Clinic in Waterbury, CT seeks a Bilingual Family Support Specialist for our Foster Care program. The role provides direct services, case management and coordination of care for an assigned caseload, advocating with DCF, schools and other supports.

Bilingual required with an annual stipend of $2,288; rate of $23.08 per hour for full-time work. Valid driver’s license required to operate in the community.

Qualifications

• BA/BS degree in Psychology, Social Work, Marriage and Family Therapy, Counseling or related field is required.
• Experience working with clients with mental health and/or co-occurring substance use disorders is preferred.
• Effective time management, strong verbal and written communication skills are required.

Responsibilities

• Develops case management plans or plans of care for assigned clients/families, coordinating care and documentation.
• Provides front line crisis support to individuals and/or families by phone, in-home, or in the community as needed.
• Interprets guidelines and adapts program procedures to address client needs and safety concerns.
• Coordinates treatment phases with internal and external service providers and family supports.
• Delivers group, individual, and/or family counseling with appropriate supervision.
• Maintains counseling and case management records in accordance with Medicaid, licensing and regulations.
