Job summary

Easterseals Florida, Inc is seeking a bilingual Family Service Coordinator to support families of young children eligible for IDEA Part C through the North Beaches Early Steps program.

You will coordinate services, participate in IFSP meetings, support eligibility and transitions, and travel to family homes to gather information and complete coordination tasks. Salary is $44,000 per year for a full-time role from 9:00 am to 5:00 pm, with benefits including insurance and paid time off.

Qualifications

• Bilingual in Spanish and English required.
• Bachelor's degree in a related field as listed.
• One year of professional experience with young children and families or similar.
• Ability to secure and maintain Medicaid Provider Number.
• Knowledge of community resources, child development, disabilities, and confidentiality laws.

Responsibilities

• Provide ongoing service coordination for Early Steps-eligible children as the assigned service coordinator.
• Participate in multidisciplinary evaluations, IFSP meetings, and transition planning activities.
• Support eligibility determinations, monitor interventions, and assist families through transitions from IDEA Part C.
• Promote IFSP team capabilities to identify, access, coordinate, and evaluate services.
• Interview families to gather information on routines, strengths, needs, and concerns.
• Develop timely IFSPs in collaboration with families and team members.
• Communicate IFSP recommendations with providers and coordinate updates every 6 months.
