Hilltop Community Resources is seeking a full-time Navigator to support families, prevent out-of-home placements, and connect clients with essential resources in Montrose, CO. The role emphasizes bilingual communication, data entry, and collaboration with community partners to foster healthy, stable families.

The position requires at least two years of college or four years of related work experience; bilingual skills are essential.

Qualifications

• Two years of college or four years of related work experience; or equivalent combination of education and experience.
• Two years of lived experience (housing insecurity, behavioral health, food insecurity or recovery from addiction) is preferred.
• CPR/First Aid certifications are preferred.
• Driver license required.

Responsabilités

• Become trained and certified in assigned Hilltop program and conduct duties per fidelity standards and program guidelines.
• Complete certifications required for the position.
• Make initial contact with participant families to present services and provide referrals.
• Create a family work file and assist families in developing their goals.
• Enter and maintain data in program databases as required.
• Refer families to external services when needed.
• Maintain contact notes with families on caseload and document interactions.
