Job summary

Sheppard Pratt is seeking a Family Case Manager for the School and Community Youth Services program in Gaithersburg, MD. This part-time, in-person role supports school-based mental health and social services by delivering culturally appropriate case management to youth and families in collaboration with MCPS and DHHS.

Responsibilities include documenting plans, maintaining regular client contact, organizing meetings, and connecting families to housing, healthcare, employment, and resources.

Qualifications

• Bachelor’s degree in Social Work (B.S.W.), Psychology, or related field. Prior experience substitutable for education.
• Bilingual fluency in English and Spanish preferred.

Responsibilities

• Provide culturally and linguistically appropriate case management services to students/parents/families with needs such as housing, healthcare, employment, food, education and other services.
• Use the County’s electronic records system to document service plans and maintain contact with clients (at least twice a month).
• Maintain a caseload in compliance with contract terms and agency expectations, and coordinate meetings and referrals to promote continuum of care.
