Job summary

Child & Family Focus, Inc. is seeking a Child/Family Case Manager to deliver supportive care to children with mental health diagnoses and their families in Montgomery County.

Responsibilities include comprehensive assessments, treatment planning, crisis support and coordination with other providers, with a strong emphasis on face-to-face service. Requires 12 credits in related coursework and 1+ year of direct clinical experience; bilingual in Spanish preferred.

Qualifications

• 12 credits in coursework related to Human Services, Psychology or related field.
• One or more years of direct clinical experience working with children, youth and families.
• Manage a varied work schedule Monday-Friday 8:30am–8:30pm, including 3 required evenings per week.

Responsibilities

• Assist in completing a comprehensive assessment and treatment plan for each client.
• Prepare environmental matrix to identify skill deficits and strengths; establish treatment goals.
• Complete written strength assessment; develop and implement written service plan.
• Provide crisis intervention to reduce unnecessary ER visits and psychiatric hospitalizations.
• Connect clients to behavioral health and social service agencies to ensure seamless services.
• Make home, school, hospital and community visits; travel across Montgomery County as needed.
• Utilize IPAD and cell phone for documentation and outreach.
• Participate in weekly team meetings and supervision.
• Achieve billable hour requirements per week.
• Maintain caseload of up to 30 clients.
• Provide 24/7 on-call crisis support in rotation.
• Apply Trauma-Informed Care core values in practice.
