Job summary

Jewish Community Services of South Florida (JCS) in Miami, FL is seeking a Family Case Manager to provide outreach, assessment, and coordination of care for individuals and families across Miami-Dade and Broward. You will develop individualized service plans, connect clients to benefits, healthcare, and community resources, and monitor progress toward goals with empathetic, culturally responsive support.

The role requires a Bachelor's degree (Master’s preferred) and Spanish fluency, with 2+

Qualifications

• Bachelor’s degree required; Master’s degree preferred in related fields.
• LCSW/LMFT/LMHC license or licensure eligibility preferred.
• Minimum of two years of relevant experience preferred.
• Fluency in Spanish required.
• Strong written and verbal communication; documentation skills.
• Proficiency in Microsoft Office; data entry essential.
• Valid Florida driver’s license and transportation required.

Responsibilities

• Complete assessments and program documents approved by Department Director.
• Develop case management treatment service plans.
• Crisis intervention and information/referrals to resources.
• Arrange for client services as needed and coordinate care.
• Monitor progress and coordinate with service providers.
• Maintain confidentiality and comply with policies and regulations.
